The Treasury has notified the court that it will be testing its Pay.gov contingency site on Saturday, March 3, 2012, between 5:00 PM and 11:00 PM.  Pay.gov will be able to process transactions normally after this planned outage. If you experience any problems with Pay.gov after the planned outages, please contact the court at 414.301.1766.

For both chapter 7 and 13 cases, you must submit the following forms and pay a filing fee before we can begin processing your bankruptcy case:

  1. Voluntary Petition (Form B1) /Sign the form and include a day-time phone number.
  2. Filing Fee or Application to Pay Filing Fee in Installments (Form B 3A) or Fee Waiver for Chapter 7 Filers (Form B 3B)
  3. Creditor Matrix /Please use our online form.
  4. Statement of Social Security Number(s) (Local Form 21) / Non-debtor spouse information must be added to this document.
  5. Certificate of Credit Counseling / (Exhibit D) Warning: You must take the credit counseling before filing bankruptcy. Please READ.
  6. Declaration & Signature of Non-Attorney Bankruptcy Petition Preparer (Form B 19) / If applicable
  7. Statement disclosing Compensation paid or to be paid to a Bankruptcy Petition Preparer (Form B 280) / If applicable

You must file the following statement within 30 days of filing the voluntary petition:

  1. Debtor's Statement of Intention (Form B8) / Chapter 7 filers only

You must file the following before a discharge can be granted:

  1. Completion of a Financial Management Course
  2. Debtor's Statement Regarding Completion of a Course in Personal Financial Management (Form 23)
  3. Chapter 13 Debtor's Certifications regarding Domestic Support Obligations and Section (Form 283)
