The United States Bankruptcy Court for the Eastern District of Wisconsin will be closed on Thursday, November 27, and Friday, November 28, 2014, in observance of the Thanksgiving holiday.

In anticipation of Chief Judge Pepper's move to the District Court, the Clerk of Courts will be reassigning her cases to other bankruptcy judges.  The Clerk expects to complete that process by December 5, 2014.  If you have a case that has not been transferred to another Judge by that date, please contact the Clerk's Office at 414-290-2701.

Creditor Forms

A Claim

A claim is the creditor's right to receive payment on a debt owed by the debtor that arose on the date of the bankruptcy filing.  See 11 U.S.C. 101 (5).  A claim may be secured or unsecured.

Proof of Claim

A proof of claim is a form used by the creditor to indicate the amount of the debt owed by the debtor on the date of the bankruptcy filing. The creditor must file the form with the clerk of the same bankruptcy court in which the bankruptcy case was filed.  Click here for a PDF fillable proof of claim.
